Clients that sent Medicaid claims to BCBSM before 1/4/2012 may receive a "A3 493 P900 Version 5010A1 not accepted for dest payer" rejection because BCBSM still had an edit in place.

Billers will have to resubmit their claims if they get this error.

BCBSM Billers that use a PO Box, and/or do not have +4 zip codes entered, who send Commercial claims in the old 4010 format may get rejections from various payers with different reasons such as "payor only accepts 5010" or "claim rejected by UnitedHealthCare status code A3:33::65".  This is because you are currently required to send commercial claims in the old 4010 format, but these payors require the new 5010 format.

We are investigating a possible change to the old 4010 format to include a physical address like 5010.

Availity may reject claims where the onset date is equal to date of service.  We are still investigating this.  Availity told a client the date should be left blank if equal, but we are not sure yet.
